All patients at Reliance Center™ are evaluated individually at the time of admission, and continually during their course of treatment with us. The Medical Directors, program counselors and therapists meet weekly to review each case and revise treatment plans and patient requirements as needed. We have found that, initially, a structured program of services and requirements makes the most sense during the first six months of treatment, since virtually all new patients go through fairly routine stages as they begin their recovery. The 'phase system' allows patients to engage in an identifiable plan of counseling and medication support that that we believe is the most effective way for patients to be transitioned from opiates to buprenorphine-assisted therapy. In addition, depending on the patient’s preference and interests, the substantial array of ancillary support services available at Reliance Center™ can also play a constructive role in helping individuals regain their health and personal wellbeing.

Phase 1 lasts 12-16 weeks and has been developed to provide the maximum number of support and counseling services in order to move the patient away from their opiate use and begin the process of medication-assisted treatment and counseling.  Services include:

• One medical evaluation including history and physical
• One comprehensive clinical assessment
• One counseling session weekly (choose from individual, family or group counseling - groups vary in format) – 12-16 sessions over 3-4 month period
• One weekly outreach call to the patient from Constant Care Connection™ clinical staff to check on status – 12-16 calls over 3-4 months
• Unlimited follow-up medication visits as needed
• Unlimited 24/7 Constant Care Connection™ telephone hotline contact with clinical staff for the client to call and speak with a counselor about patient problems, issues and concerns
•  Unlimited 24/7 texting with counseling staff
• One free ancillary service (massage, yoga, acupuncture, chiropractic)
• Unlimited ancillary services on a fee-for-service basis

 

Phase 2 lasts 12-16 weeks and has been developed to assist the patient in the process of developing a strong and resilient personal recovery program on a reduced commitment basis.  Services include:

• One counseling session bi-monthly (patient choice of format) – 6-8 sessions over 3-4 months
• One weekly outreach call from Constant Care Connection™ clinical staff to the patient to check on status – 12-16 calls over 3-4 months.
• Unlimited follow-up medication visits as needed
• Unlimited 24/7 Constant Care Connection™ telephone hotline for the client to call and speak with a counselor about their problems, issues and concerns
• Unlimited texting 24/7 with clinical staff
  
• Unlimited ancillary services on a fee-for-service basis

 

Phase 3 does not have a specific time duration; it has been constructed to allow patients to continue to receive services from Reliance Center™ on an unrestricted basis as they continue to rebuild their personal and professional lives.  Services include: 

• One counseling session (patient choice) monthly
• Unlimited follow-up medication visits as needed (minimum of one monthly)
• Bi-monthly outreach call to the patient from Constant Care Connection™ clinical staff to check on status
• Unlimited 24/7 Constant Care Connection™ telephone hotline for the client to call and speak with a counselor with problems, issues and concerns
• Unlimited texting 24/7 with clinical staff
  
• Unlimited ancillary services on a fee-for-service basis
